Attendance Record:  Must mail in a copy of this form once
per quater.  Send final report at the end of the school year.
Attendance Online Input:  Quarterly online reporting in lieu
of paper copy, but must mail in final report at end of year.
High School Semester Progress Report:  Must mail in a
copy after each semester.
Grade School Semester Progress Report:  Must mail in a
copy after each semester.
Report Card:  Optional report card can be mailed in after
each semester for all grades in replacement of the progress
report.  Graded report cards are required for all high school
students.

Non-Regina Caeli Families who privately home school are free to
copy the basic format of these forms.

Registered families may request a free CD with all the forms in Microsoft
Word format for electronic editing.  Otherwise these printed forms are
acceptable with legible writing.
